Adds CodeyBox:CommitAttribution (CoAuthoredBy, CodeyBoxTrailers, PullRequestFooter; all default true) plus per-project overrides resolved through a single injected CommitAttributionPolicy. Disabled lines are omitted from orchestrator/mechanical/conflict/merge commits, agent prompts stop requesting them, the host strips agent-added ones by exact key, the GitHub PR footer and squash trailers follow the same policy, and the prompt-revision auditor is skipped with an explicit reason while stale-prompt tracking continues from the DB dispatch ledger.
